A.Introductory letter for review. Refer to this Section, paragraph 3.1 C.
1.Introductory letters including descriptive content specific to an individual property owner for separate review.
B.Log of properties contacted including the address, parcel number, names of the individuals contacted, description of attempts at contact, and results of contact , for information .
C.Documentation for each property surveyed for information . Include at least the following:
1.Video surveys in electronic format with accompanying audio.
2.Associated documentation with the date, time, street address, parcel number, names of people preforming the survey and names of owners or owners’ representatives present during the survey.

Part 3 — Execution
3.1 Preconstruction Survey
A.Before starting construction activities, perform an existing condition survey of designated adjacent properties to accurately document the existing conditions.
B.Accompli sh the survey under the dir ect supervision of a Utah licensed profession al engineer familiar with construction work and practices.
C.Gain permission from the property owner for access to private property to document preconstruction conditions.
1.Send a certified, return receipt requested introductory letter (wording to be reviewed by the Department) to inform the property owner of the s urvey and to request permission to survey the existing conditions.
2.Notify the Department and log all contacts with the property owner if the proper ty owner refuses access .
D.Use the public involvement representative to coordinate the survey with the property owner.
1.Verify the representative is present during the survey.
E.Document and video -record the existing condition for each property surveyed , clearly and precisely showing and describing existing damage.
1.Provide continuous footage of the entire duration spent on private property.
2.Describe the lengths and widths of existing cracks in the video recording.
3.Survey at least the following on each property:
a.Foundations
b.Exterior walls, interior walls and ceilings of buildings
4.Retai ning walls
5.Driveways
6.Sidewalks
7.Swimming pools
8.Parking lo ts
9.Other auxiliary structures and features that may be at risk of damage due to construction activities.
