The letter of procedure must incorporate all manufacturer recommendations for installation. Along with the letter of procedure submit a sketch with enough detail to support the Post-Installed Anchor Designer’s calculations . Submit an inspection procedure that includes how the inspection will be completed by a certified anchor inspector and how often the inspections are to be performed. Also, submit the following information:
• Product name, type, diameter, and length.
• Product specifications with recommended design values and physical characteristics.
• Samples: Submit samples to LTS before incorporation in the work . Representative length and diameters of each type of anchor used on the project . Test as specified in Section 698.2(e).
• Test Reports: Certified test reports showing compliance with specified performance characteristics and physical characteristics.
• Most recent ICC -ES or IAPMO -UES Evaluation Report(s) - ESRs.
• MPII.
• Product name, type, diameter, and length.
• Product specifications with recommended design values and physical characteristics.
• Type of anchor installed in the adhesive and its mechanical and physical characteristics.
• Samples: Submit samples of the threaded rod or reinforcing bar insert to LTS before incorporation in the work . Representative length and diameters of each type of anchor used on the project . Test as specified in Section 698.2(f).
• Test Reports: Certified mill test reports showing compliance with specified performance characteristics and physical characteristics used in the manufacture of the anchors.
